TESTIMONIAL CONSENT AND RELEASE
AUTHORIZATION TO USE NAME, LIKENESS AND VOICE

I hereby expressly grant "your company", Responsive Nutrition Inc. (collectively referred to as "Company"), as well as their respective affiliates, assigns, partnerships and joint ventures, the unrestrictive right and permission to use any and all of my statements, testimonials, still or moving photographic images and endorsements ("Statements") as well as my first name only, likeness and/or voice recordings (collectively referred to as "Likeness") in reference regarding "your company" ("The Product" or "Product").

I agree to grant to Company worldwide rights in perpetuity for my Likeness to be utilized in all media and communication vehicles including without limitation television, cable, satellite, radio, print, internet advertising, brochures, and public relation purposes, as well as all forms of media now known or later invented ("Media"). I understand that I am not being paid any compensation for the use of my likeness now, or at any time in the future but will be reimbursed for reasonable out-of-pocket expenses, if any. I hereby forever expressly waive, release and relinquish all of my rights, title and interest in and my likeness being used in all of the Media.

I realize that the Company has relied at their expense and potential detriment upon my commitment to grant all of the uses set forth herein.

I represent and warrant that I have personally used the Product and all of the statements I have made are my own opinions based upon my own experience. I represent and warrant that I am over eighteen years of age, and competent to contract in my own name. I have read this testimonial release, and warrant that I fully understand the contents thereof. Further, I represent and warrant that I am not a member of any union including without limitation SAG and/or AFTRA that would have any jurisdiction over this agreement.

This is the entire Agreement between the parties hereto and may not be modified in any manner. Should any legal proceeding be commenced regarding this Agreement, the prevailing party shall be entitled to such relief as granted, plus reasonable attorney fees and costs. Jurisdiction for any legal proceeding shall be in California, with venue in Los Angeles County.

BY CLICKING ON THE "I ACCEPT", I WARRANT THAT I HAVE CAREFULLY READ THE ABOVE TERMS AND HEREBY AGREE TO, AND ACCEPT ALL OF THE ABOVE TERMS.
